Mediation Forms


Please call (701) 484-2224 or email to speak with staff before completing any forms.

After you have spoken with staff, please do the following:

  1. Complete the Agreement to Mediate (link below)

  2. Complete the Mediation Intake Form (link below)

  3. Pay your share of the mediation retainer (link below or mail a check or money order)

Unless otherwise arranged, all mediation sessions take place by video conference.

You will receive an email confirming your appointment. The email includes the Zoom link you will need to use to log on for our session. See below for more information about using Zoom.
